What Is the Science Behind Stem Cells?

Stem cells are the body’s natural repair kit. Unlike specialized cells (like skin or muscle cells), stem cells have the unique ability to transform into almost any type of cell. Think of them as blank slates that can specialize to regenerate damaged tissues.

In regenerative medicine, these powerful cells are harvested, processed, and then injected into the damaged area—whether it’s a joint, disc, or surrounding tissues. Once injected, stem cells get to work by promoting cellular repair, reducing inflammation, and stimulating the growth of new, healthy tissue. Unlike treatments that simply mask pain, stem cells aim to fix the underlying issue.

🌱 Learn more about the regenerative power of stem cells in our exclusive interview with Dr. Isabella Bernaschina.

How Does Stem Cell Therapy Work for Back Pain?

For back pain, the underlying issues often include degenerative disc disease, joint damage, and nerve inflammation—all of which stem cell therapy can target.

Unlike traditional treatments that rely on medication or invasive procedures, stem cell therapy addresses the root cause of the pain and can even help avoid surgery. Here’s how it works:

🩹 Tissue repair and regeneration

Chronic pain often arises from degeneration or injury of spinal discs, joints, or soft tissues like muscles and ligaments. Stem cells, particularly mesenchymal stem cells (MSCs), have the unique ability to differentiate into various cell types, such as:

  • cartilage cells (chondrocytes);
  • bone cells (osteoblasts);
  • muscle cells (myocytes).

When injected into a damaged area, stem cells can potentially develop into the type of cells needed to repair damaged tissues, such as intervertebral discs or facet joints.

🍃 Reducing nerve inflammation

Stem cells release bioactive molecules like cytokines and growth factors that modulate the immune response. They help reduce inflammation, which significantly contributes to chronic back pain.

🧫 Stimulation of local cells

Stem cells also function by releasing paracrine factors, which stimulate the body's own cells to start repairing the damaged tissue. Thus, the treatment enhances natural healing processes, encouraging local cells in the spine to regenerate faster and more effectively.

🎥 Watch the story of a patient who found relief from back pain with stem cell therapy after undergoing multiple spinal surgeries:

Back conditions treated with stem cells

The main back conditions that can benefit from stem cell treatment are:

1️⃣ Degenerative disc disease (DDD)
Degenerative disc disease is one of the most common causes of chronic back pain, occurring when the discs between the vertebrae break down, lose flexibility, or compress. Stem cells can help regenerate damaged discs, reducing pain and improving function by encouraging the growth of new, healthy tissue.

2️⃣ Facet joint arthritis
Facet joints are small joints that connect the vertebrae. When these joints become worn or inflamed (facet joint arthritis), they can cause significant back pain and stiffness. Stem cells can regenerate cartilage, helping to relieve joint pain and improve movement.

3️⃣ Sciatica
Sciatica occurs when the sciatic nerve is compressed, often due to herniated discs or other spinal issues, leading to radiating pain down the legs. Stem cell therapy may help by reducing inflammation and promoting the healing of damaged discs or surrounding tissues.

4️⃣ Spinal stenosis
Spinal stenosis is a condition where the spinal canal narrows, putting pressure on the spinal cord and nerves. This condition can lead to pain, numbness, and difficulty walking. Stem cell injections can help reduce inflammation and slow down the degenerative process.

5️⃣ Herniated discs
A herniated disc occurs when the soft tissue inside a spinal disc pushes out, compressing nearby nerves. Stem cells can be injected into the affected area to promote tissue healing and reduce nerve compression.

6️⃣ Sacroiliac joint dysfunction
The sacroiliac joint connects the lower spine to the pelvis and can become inflamed or misaligned, leading to chronic pain. Stem cells can help regenerate tissue in the sacroiliac joint, reducing pain and improving stability in the pelvic region.

7️⃣ Osteoarthritis of the spine
Osteoarthritis can affect the spine, causing wear and tear on the joints and discs. Stem cell therapy can regenerate cartilage and reduce inflammation, potentially slowing the progression of osteoarthritis and relieving chronic pain and stiffness.

8️⃣ Spinal cord injuries
Stem cell therapy for spinal injuries holds significant promise, especially for conditions like spinal cord injury (SCI), where damage to the nerves can lead to paralysis or loss of motor and sensory functions. Clinical trials have already shown significant improvements and no adverse effects for people with spinal cord injuries and paralysis.

Types of stem cells used for back pain

Mesenchymal stem cells (MSCs) are the most common stem cell type used to treat back pain. These cells can transform into multiple tissue types, including bone, cartilage, and muscle. They can be sourced from:

  • Bone marrow (BM-MSCs). Bone marrow stem cells are especially popular because they are easy to access and isolate. They successfully repair and regenerate damaged discs and joints.
  • Adipose tissue (AD-MSCs). These cells are abundant in fat tissue and are even easier to obtain. Studies have shown that AD-MSCs can effectively regenerate disc tissue, potentially even better than bone marrow-derived MSCs.
  • Umbilical cord blood (UC-MSCs). Umbilical cord blood is a non-invasive option with promising results in improving pain and mobility. They are collected from the umbilical cord after childbirth. Stem cell collection is entirely ethical since it is painless for both the mother and the baby.

In addition to MSCs, researchers are currently exploring other types of stem cells for spinal regeneration:

  • Nucleus pulposus progenitor cells (NPPCs) are stem cells maintaining the health and function of spinal discs.
  • Embryonic stem cells (ESCs) are pluripotent cells from embryos capable of differentiating into any cell type.
  • Induced pluripotent stem cells (iPSCs) are adult cells reprogrammed to behave like embryonic stem cells.

How are stem cells administered for back pain?

Stem cell treatment for back pain follows a multi-step process:

1. Stem cell collection

Stem cells can be collected from the patient (autologous) or a stem cell bank (allogeneic). The specific process depends on the source:

  • Bone marrow: Doctors extract stem cells using a needle, typically from the hip bone (iliac crest).
  • Fat tissue: Stem cells are collected through liposuction.

2. Stem cell preparation

Once collected, the stem cells are processed to ensure they are ready for injection:

  • Centrifugation: For sources like bone marrow and fat tissue, a centrifuge machine spins the cells to concentrate growth factors and other healing elements.
  • Lab preparation: Umbilical cord and other donor-derived stem cells are prepared in a lab for immediate use, ensuring they meet medical standards for injection.

3. Stem cell administration

After the preparation, doctors inject stem cells into the affected area. The most common methods include:

  • Intradiscal injection: This involves directly injecting stem cells into a damaged or degenerated spinal disc. It is a localized, minimally invasive procedure.
  • Central line infusion: Stem cells are delivered through a central line into broader areas of the spine or body. The medical team uses fluoroscopy (special X-ray guidance) to ensure precise placement. This technique works for more generalized spinal conditions or when targeting multiple areas.

🤔 What about the recovery?

Stem cell therapy is a minimally invasive procedure. Patients may experience temporary soreness and stiffness in the treated area. Full recovery and noticeable improvements can take up to 12 months as the stem cells work to repair and regenerate the damaged tissues over time.

The Cost of Stem Cell Therapy for Back Pain

Stem cell therapy cost can vary widely, as it is a highly personalized treatment. Different factors, like the patient’s condition, the type of stem cells used, and the number of injections, play a role. On average, stem cell injections cost from $5,000 to $8,000 for patients with back problems.

🗽 In the U.S., stem cell therapy is still undergoing clinical trials and has yet to be approved by the FDA for chronic back pain. As a result, reliable treatments are not widely available and can cost much higher—up to $15,000 for a single injection. Additionally, limited access requires patients to wait longer for treatments or settle for higher out-of-pocket procedure expenses.

✈️ Why consider treatment abroad?

Countries like Mexico, Colombia, Turkey, and Thailand have well-regulated stem cell therapies approved by their respective health authorities and FDA equivalents. This wider access to stem cell treatments leads to significantly lower prices.

A single stem cell injection can start at $1,000 abroad, compared to $4,000 in the U.S. Comprehensive packages that include consultations, diagnostics, and follow-up care range between $7,000 to $10,500.

LIV Hospital services are very much in line with and in no way inferior to the world level, believe me, because due to health problems of my family, we had to see clinics in the USA, China, South Korea, Israel. From the very first call I felt great care for me as a loved one. Many thanks to Sabina, Nigina - coordinators / translators, they did a great job, we could contact them at any time of the day and they supported the patient with all their hearts. Please note that everything here is based on Turkish, only doctors can communicate in English. It can be seen how much Turkey has advanced and is self-sufficient. I am grateful to fate that I met the professor - neurosurgeon Chagatay Ozturk. Easy to communicate, concise, it is immediately clear that the highest level specialist, after the examination, identified the problem and showed it on the MRI scan and explained what was to come. An operation was performed to remove the hernia on L5-S1 and a titanium disc was installed. The atmosphere of the team was very favorable and cozy, there was a high level of coherence and discipline. The concern was such that I did not feel any pain or heaviness, although they say that the operation was not an easy one. On the second day after the operation, I already walked and on the sixth day I was already discharged. Finally I got rid of pain and sleepless nights over the years! I recommend to everyone, wizards work here - professionals in their field! Best regards, Gaziz Kissym Kazakhstan

At first I was hesitant for the treatment in Mexico but after the meeting with doctor Isabella, my entire perspective changed. She is very knowledgeable, professional and personable. She will explain the treatment and procedure with you in detail. I will not hesitate again to recommend her to any of my friends and family. She will perform the procedure in the comfort of your own residency without having to travel to the clinic. This is very accommodating as it minimizes the interruption if or when you are on vacation. I will be following up with her in the coming months as she is now my go to doctor.

Hello, my name is Vladislav Ivanov, Ukraine, Odessa city. In February 2021, my daughter Vlada was diagnosed with anaplastic ependymoma, we went to the doctors for a week trying to find out why the child started having severe headaches. They went through examinations, did an MRI, and as a fact, they found a brain tumor. That's when the ground slipped from under my feet. Realization has come - this has already happened, which means you need to quickly take action. The first thing we did was go to the children's hospital in Kyiv, the child was given a shunt. But unfortunately, fluid began to collect in my head and my vision began to fall sharply. The doctors explained that an operation was urgently needed. We did not have a large accumulated amount, and we decided to complete the operation abroad. We contacted Bookmed. We contacted the employee Marina. Marina quickly found a clinic for us in Turkey, organized a trip for us, flew to Turkey in a matter of days. The hospital was already waiting for us. Examined, took blood for tests and the next day an operation was scheduled. Everything went well. Then they underwent a course of radiotherapy. The results are positive, we were sent home to Ukraine (August 2021). Every 3 months they underwent MRI, the pictures were good .. But a year later, in March 2022, we heard the terrible words “relapse”, “war”. Hastily collected the necessary documents and left for Lviv. From there, by large buses, all children with cancer began to be taken out along the humanitarian corridor from Ukraine to Europe. The daughter and his wife were assigned to France. A second operation was performed, the tumor was removed, and after that a course of chemotherapy was prescribed. After a month of taking the course, the doctor said that chemotherapy was not helping. The doctor suggested experimental chemotherapy, but that didn't help either. As a result, the daughter is getting worse and worse every day. The child began to walk badly, the right leg and handle practically do not work. The child melted before my eyes. It's all very scary, but it's worse to leave Vlad without an opportunity for treatment. I again turned to Marina Bookmed Marina again very quickly found us a hospital in Turkey, helped us with the flight Paris Istanbul. Arrived at the hospital 2 days later, the operation was successful!!! After the operation, the child began to talk, swallow, and after a couple of days to walk. What I want to say Bookmed in particular Marina Provided us with moral support documentary for the second time at the highest level. Marina many thanks to you and your team THANK YOU.

🌍💵 Find out more about the costs of stem cells therapy and the best destinations.

Does Medicare Cover Stem Cell Therapy for Back Pain?

Like other major insurance providers in the U.S., Medicare does not cover stem cell therapy for back pain. This is because the FDA has not yet fully approved stem cell treatments, classifying them as experimental. As a result, patients seeking these therapies usually need to pay out of pocket.

However, as stem cell research progresses, there’s potential for broader insurance coverage in the future. Until then, exploring more affordable options abroad may be necessary.

Factors Affecting Stem Cell Therapy Cost

Stem cell therapy cost can vary greatly depending on several factors:

1. Clinic’s reputation and expertise

Well-established clinics with experienced medical teams often charge more, but they provide a higher level of care and safety. Choosing a reputable clinic is crucial for getting the best outcomes, especially with newer treatments like stem cell therapy.

2. Location of the clinic

Where you receive treatment has a significant impact on the cost. In the U.S., treatments are significantly more expensive due to FDA restrictions and limited availability. In contrast, countries like Mexico, Colombia, Turkey, and Thailand offer the same treatments for much lower prices, as their regulatory frameworks allow broader access to stem cell procedures.

3. Patient’s condition

The severity and extent of the patient’s condition will affect the cost of treatment. Stem cell injections for localized pain are less expensive than comprehensive treatments for more widespread or severe issues.

4. Number of treatments and stem cell count

Back pain treatments may require multiple stem cell injections to achieve optimal results. The more sessions needed, the higher the total stem cell treatment cost. Treatments involving higher stem cell counts are also more expensive.

5. Type of stem cells used

The source of stem cells plays a significant role in determining the cost. For example, stem cells harvested from your own body tend to be more expensive due to the extraction and isolation process. Donor stem cells may cost less because they don’t require invasive harvesting procedures.

6. Pre- and post-treatment care

The cost of stem cell therapy can also vary depending on the inclusions of your treatment package. Bookimed’s partner clinics often have all-inclusive packages that cover consultations, diagnostics, injections, and follow-up care. On the other hand, other medical providers may charge separately for these services, increasing the overall price.

What Is Included in the Stem Cell Treatment Cost?

The overall cost of stem cell therapy involves several essential components. Here is what you’re typically paying for:

1. Stem cell extraction or preparation

If you're using autologous stem cells (from your own body), the cost includes the extraction process, such as bone marrow aspiration or liposuction. For allogeneic (donor) stem cells, the price includes the procurement and preparation of the cells.

2. Stem cell processing

Once harvested, the stem cells are processed in a lab to prepare them for safe and effective injection.

3. Stem cell injection therapy

This cost covers the doctor’s time, the use of imaging technology (such as fluoroscopy) to guide the injection, and any anesthesia or other support required during the procedure.

4. Basic follow-up care

Most treatments include some level of follow-up care to monitor recovery and evaluate the effectiveness of the therapy. However, this is usually limited to a few initial visits, and additional follow-up may come at an extra cost.

Additional costs

Beyond the basic elements, stem cell therapy involves some additional expenses:

  • consultations and diagnostics;
  • repeat stem cell treatment sessions;
  • post-treatment medication;
  • accommodation and travel.

How to Reduce the Cost of Stem Cell Therapy?

While stem cell procedures can be expensive, there are ways to reduce costs without compromising quality. For instance, Bokimed’s partner clinics in Mexico, Colombia, Turkey, and Thailand offer all-inclusive regenerative medicine packages. They bundle everything into one price, making it easier to budget and avoid unexpected costs. Here’s what they can include aside from the procedure itself:

  • 👨‍⚕ Consultations and diagnostics. Packages include initial consultations with specialists and any necessary diagnostic tests, so you don’t have to pay separately for these.
  • 💉 Treatment sessions. The package includes stem cell extraction, preparation, and injection, often at a fixed price that can be more cost-effective.
  • 🛌🏽 Post-treatment care. Packages may include extended follow-up visits. Some clinics even offer consultations with a dietitian to help support recovery with a specialized diet.
  • 🏩🚘 Accommodation and transfers. For international patients, clinics include hotel stays, airport transfers, and transportation to and from the clinic, taking care of travel costs.
  • ➕ Additional treatments. Some packages offer added therapies like platelet-rich plasma (PRP) to enhance tissue healing or oxygen therapy to improve circulation and recovery after treatment.
  • 🔖 Discounted rates for repeat treatments. Preferential pricing on additional stem cell injections ensures long-term savings if further treatments are needed.

Success Rates of Stem Cell Therapy for Back Pain: Research and Clinical Trial Results

Regenerative medicine research and clinical trials have shown promising results in the effectiveness of stem cell therapy for back pain. Here are some key findings from various studies:

🩹 Reduction in pain and improved function

In one study, 69% of patients who received bone marrow stem cells for lumbar disc degeneration reported a ≥50% reduction in pain after 12 months.

🔄 Positive outcomes in disc regeneration

In another study, 85% of patients showed a reduction in disc bulge size by an average of 23%. Improvement in the spine's structural integrity was observed even 6 years after the treatment.

📈 Sustained improvements over time

A series of case studies followed 10 patients who received bone marrow stem cells for chronic back pain. It found rapid improvements in pain and function at 3 months, with continued gains over 12 months.

🛡️ Long-term safety

Stem cell therapy has also proven to be safe, with very few adverse events reported in studies spanning up to 6 years. Minor side effects like headache and musculoskeletal pain resolve with simple over-the-counter medication.

⚡Improved sensation and motor function for spinal injury patients

In a Mayo Clinic study, 70% of participants with a traumatic spinal cord injury showed improvements on the American Spinal Injury Association (ASIA) Impairment Scale. The participants moved up at least one level on the scale, which measures sensory and motor function recovery. Improvements included better sensation, increased muscle strength, and improved voluntary anal contraction.

How to Find the Best Stem Cell Clinic for Back Pain

Choosing the right stem cell clinic for back treatment is the most crucial step. Your results heavily depend on the quality of care and expertise. Here are some practical tips to help you find the best stem cell clinic:

1️⃣ Check certifications and accreditations

Look for stem cell clinics with recognized accreditation from health organizations like JCI (Joint Commission International) or ISO certification. They ensure the clinic follows international safety and quality standards.

2️⃣ Evaluate the doctor’s expertise

Look for doctors experienced in stem cell therapy and spine health, particularly those with a proven track record of treating chronic back pain.

3️⃣ Ask for success rates and patient testimonials

A reputable clinic should be able to provide you with data and patient testimonials that speak to the effectiveness of their treatments. This will give you confidence in the clinic’s ability to deliver results.

4️⃣ Inquire about safety protocols

Ask how the clinic retrieves, processes, and stores stem cells. Ensure they follow strict safety protocols. Clinics should use high-quality labs for cell preparation and have transparent guidelines to avoid contamination or mishandling.

5️⃣ Understand the full scope of treatment

Some stem cell clinics offer all-inclusive packages covering everything from consultations and diagnostics to follow-up care and additional treatments. These packages help reduce overall costs while ensuring that you receive comprehensive care.

6️⃣ Consider the clinic’s technology and facilities

Look for a well-equipped stem cell clinic with modern diagnostic tools, such as MRI machines, fluoroscopy, and a lab for cell processing. Advanced facilities ensure that your stem cells are handled and administered with precision.

7️⃣ Inquire about post-treatment support

The best clinics offer follow-up consultations and remote monitoring to ensure your recovery is on track.

8️⃣ Pay attention to patient comfort

While it might seem less important, patient comfort can make a big difference in your treatment experience. Clinics offering personalized services, such as private recovery rooms and personal coordinators, provide a better experience.

9️⃣ Avoid clinics making unrealistic promises

A trustworthy clinic will set realistic expectations and explain the potential risks and benefits without overselling the treatment.
