In the case of autologous bone marrow transplantation, you will receive specific medications to stimulate your stem cell production for further collection. If you require an allogeneic transplantation and a donor is available, doctors will gather donor cells.

Chemo- and/or radiotherapy cycles

Before the bone marrow transplant, a patient undergoes high-dose therapy (sometimes with total body irradiation) to prepare the body for the better adoption of new cells.

  • destroy malignant cells in the case of cancer diagnosis;
  • destroy existing bone marrow cells to free space for new healthy ones;
  • suppress the immune system so that it doesn’t reject the transplant.

Transplantation process

The transplant typically happens several days after the conditioning treatment is complete. The BMT procedure is painless and can take anywhere from 30 minutes to 4 hours. Healthy stem cells will be gradually infused into the vein through a catheter. During the procedure, the patient remains awake, and no anesthetic solution is needed.

Post-op recovery

Once new bone marrow cells enter your body, they travel through the bloodstream, reach the bone marrow, and start multiplying. In several weeks, the number of healthy stem cells will reach a required level. 

Throughout the first 100 days after the transplant, you should anticipate frequent clinic appointments to monitor the state of your health. They will include various tests and physical examinations. During this time, the following procedures might also be required:

  • administration of fluids through your mouth or a nasogastric tube to avoid malnutrition;
  • blood and platelet transfusions to increase the number of red blood cells and platelets in the body.

Your immune system might need over a year after the transplantation to get strong again, so you’ll be at risk of infections during this time. In the case of allogeneic transplantation, doctors often prescribe immunosuppressants. These drugs weaken the immune system, preventing it from fighting transplanted cells.     Are bone marrow transplants painful for the patient or donor?

    The procedure is not painful for the recipient, doesn’t require the use of anesthetics, and takes only a couple of hours.

    As for the donation, bone marrow stem cells are harvested under a general (or sometimes local) anesthetic. Therefore, the donor will not be awake during the procedure and won’t feel any pain. After the collection of stem cells, it is normal to experience pain in the area where the needle was inserted.     • PBSC donation: 90% of donors need to attend only 1 session lasting up to 8 hours; the remaining 10% will need 2 sessions, each lasting 4-6 hours. During the procedure, the donor is connected to a blood cell separator device via tubes in both arms. After blood-forming cells are collected from one arm, the remaining blood returns to your body via a needle in the other arm.
    • Marrow donation: it's a surgical procedure conducted under general anesthesia. The graft is taken directly from the bone marrow since it is a rich stem cell source.     Can a bone marrow transplant change your blood type or DNA?

    Yes, after the allogeneic transplant, the recipient’s blood type changes into the donor's. This happens because bone marrow produces most of the body’s blood cells. When the patient undergoes BMT, donated blood cells replace the damaged cells and begin to produce new ones. So, if your donor has a different blood type, your own blood will eventually convert to it.

    To some extent, hematopoietic stem cell transplantation can also change your DNA. If a person receives an allogeneic transplant, the new stem cells will have the donor’s DNA. This phenomenon is known as chimerism. In Greek mythology, the creature Chimera was partly a lion, partly a goat, and partly a snake. On the other hand, a person with chimerism will have the donor’s DNA in their blood and their original DNA in most other cells. Furthermore, recent studies suggest that donor DNA may migrate to other body parts.
