"I can safely recommend this doctor, operations are performed here every day, everything is thought out to the smallest detail and you do not care, as in Moscow, for example, what equipment or conditions for treatment are lacking
I'll tell you how it all happened, maybe it will help someone make their choice. I'll start with the fact that I was looking for a clinic for a long time to treat my son both in Europe and in Russia. The son is 5 years old, it was necessary to carry out a radical operation on the aortic valve, plastic or Rosa operation. The second is very difficult to perform, so it was important to find a surgeon with extensive experience. Alessandro Frigiola has a lot of experience. In Moscow, there are generally few reviews about Italian medicine, but after visiting here I can safely recommend this doctor, operations are performed here every day, everything is thought out to the smallest detail and you do not care, as in Moscow, for example, what equipment or conditions for treatment are lacking. I liked the San Donato clinic itself, despite the fact that almost no one speaks Russian, language barriers almost did not interfere, the staff are caring, they treat children with love. The atmosphere is pleasant here. Special thanks to the staff of international treatment organizations, the department may not write correctly, but I think it's clear. For all questions 24/7, you can contact them, Olga Gribova helped organize everything for a whole year. An excellent translator, Mariana, who also helps to communicate with doctors in the clinic and is constantly in touch if any problems arise. For all questions, even those that did not concern treatment, the girls always responded very responsively and helped. Thanks to them, the trip turned out to be much easier morally and with less worries. I was allowed to visit my son in intensive care. He stayed there for 2 days, and could have stayed less, but there were delays due to a smear on the covid, so they decided to keep him there, because the operation was difficult. Accommodation here is separate from the hospital, but Olga also helped with this issue. Everything was organized very quickly, there is no red tape with documents, as, for example, in Germany. As for the ward itself, in principle, everything is like everywhere else, I have no particular complaints.

Expert Overview about Aortic valve insufficiency Treatment in Italy

These FAQs come from real patients seeking medical assistance through Bookimed. Answers are given by experienced medical coordinators and trusted clinic representatives.

What are the main surgical options for treating aortic insufficiency in Italy?

Italy offers advanced aortic insufficiency treatments ranging from minimally invasive valve repair to complex reconstructions. Leading Italian centers prioritize native valve preservation using reconstructive plastic surgery or valve-sparing root replacement. Specialized hospitals like San Donato Hospital perform over 9,400 cardiac operations annually.

  • Valve replacement: Surgeons use mechanical or bioprosthetic valves based on patient age and lifestyle.
  • Aortic valve repair: Specialists reshape native leaflets, avoiding lifelong anticoagulants for younger patients.
  • Valve-sparing replacement: Procedures like the David Operation treat dilated roots while preserving healthy valves.
  • Ross procedure: Specific centers replace the diseased aortic valve with the patient's own pulmonary valve.

Bookimed Expert Insight: Italian cardiac care is highly centralized, with Maria Cecilia Hospital alone handling over 15% of the country's heart surgeries. This high volume allows for niche specializations, such as endovascular brain protection during TAVI procedures performed by experts like Dr. Fausto Castriota. Choosing these high-volume centers often provides access to multidisciplinary heart teams that specialized in both open and transcatheter methods.

Patient Consensus: Patients emphasize the importance of finding a high-volume center for complex reconstructions like the Ross procedure. They note that Italian clinics offer quick organization with minimal paperwork compared to other European hubs.

Does Italy follow the same medical guidelines as the US for timing valve surgery?

Italy follows European guidelines which parallel US standards but differ in intervention thresholds for asymptomatic patients. Italian centers use a 55% ejection fraction trigger for surgery. US centers wait for 60%. Italy also favors traditional surgery for patients under age 75.

  • Guideline body: Italy follows the European Society of Cardiology (ESC) regulatory standards.
  • TAVI age: Transcatheter options are prioritized for patients 75 and older in Italy.
  • Valve type: Mechanical valves are recommended for patients under 60 in Italian centers.
  • Follow-up care: Specialized clinics emphasize serial imaging to track ventricular trends over time.

Bookimed Expert Insight: Italian centers like San Donato Hospital perform over 9,400 cardiovascular operations annually. Our data shows a high concentration of expertise in Milan. This volume allows surgeons like Dr. Giuseppe Speziale to specialize in niche areas. These include minimally invasive repairs that traditional guidelines may not fully capture yet.

Patient Consensus: Patients note that Italian doctors often take a more conservative approach. Many report that serial testing is vital to catch subtle drops in energy before surgery.

Is Transcatheter Aortic Valve Implantation (TAVI/TAVR) an option for isolated aortic insufficiency?

Transcatheter aortic valve implantation (TAVI) is an emerging option for isolated aortic insufficiency in patients facing high surgical risks. While technically demanding due to low calcification, specialized anchoring devices now allow for securing prostheses in non-calcified valves within leading Italian cardiac centers.

  • Patient eligibility: Reserved for elderly or high-risk cases where open surgery is unacceptably dangerous.
  • Technical challenge: Smooth valve leaflets make friction-anchoring difficult compared to traditional aortic stenosis.
  • Dedicated devices: Systems like J-Valve use specialized clips to anchor directly onto valve leaflets.
  • Clinical evaluation: Multidisciplinary heart teams use CT scans to assess annulus size and root dilation.

Bookimed Expert Insight: Italian centers like San Donato Hospital and Maria Cecilia Hospital maintain a significant edge in complex valve cases. Maria Cecilia Hospital alone handles over 15% of all cardiac surgeries in Italy. This high volume is critical for TAVI in aortic insufficiency because the procedure often requires off-label techniques or specialized anchoring systems. Dr. Fausto Castriota at Maria Cecilia Hospital is a leading expert in brain protection during these valve procedures. Choosing a high-volume center with researchers like Dr. Castriota ensures access to the latest protective protocols during complex TAVI implantations.

Patient Consensus: Patients emphasize finding surgeons with extensive experience in both plastic repair and radical valve replacement. They note how helpful it is when doctors, like Dr. Maisano Francesco, take the time to explain the specific anatomical reasons for choosing one valve approach over another.

Do Italian hospitals provide English-speaking support for international patients?

Italian private hospitals and large university centers provide English-speaking support through dedicated international patient offices. Facilities like San Donato and San Raffaele in Milan employ multilingual coordinators. While senior specialists usually speak English fluently, nursing and administrative staff in smaller regional public hospitals often have limited proficiency.

  • International departments: Private centers maintain offices for translation, insurance billing, and appointment coordination.
  • Doctor communication: Senior cardiologists and surgeons frequently hold international credentials and speak fluent English.
  • Administrative gaps: Front-desk and nursing staff in public facilities may require translation apps.
  • 24/7 assistance: The 116117 hotline provides medical guidance in English across major Italian regions.

Bookimed Expert Insight: Italian cardiac centers like Maria Cecilia Hospital and San Donato handle massive patient volumes, with some performing over 9,400 surgeries annually. Our data shows that these high-volume facilities prioritize international coordination because they manage thousands of foreign patients. Choosing a center with JCI accreditation usually guarantees a structured international department that handles everything from medical records to bedside translation.

Patient Consensus: Patients note that while surgeons like Dr. Maisano Francesco explain procedures clearly in English, having a dedicated translator is vital for daily ward interactions. Families emphasize that international departments are essential for handling logistics and communicating with the medical team 24/7.

How long should I plan to stay in Italy for treatment and initial recovery?

Patients should plan to stay in Italy for 14 to 21 days for aortic valve treatment. This window ensures safe recovery before boarding long-haul flights. Duration depends on whether surgeons use transcatheter aortic valve implantation (TAVI) or open-heart surgery. A physician-issued `Fit to Fly` certificate is mandatory for international travel.

  • Procedure duration: Most aortic valve surgeries require 5 to 7 days of inpatient hospital care.
  • Minimally invasive TAVI: Endovascular techniques like TAVI often allow for shorter 7 to 10-day local stays.
  • Post-operative monitoring: Plan for 7 days of local lodging after discharge for essential follow-up imaging.
  • Physical milestones: Patients must walk independently and manage stairs before surgeons grant final travel clearance.

Bookimed Expert Insight: Milan serves as Italy's primary cardiac hub, with San Donato Hospital alone performing over 9,400 heart operations annually. Research hospitals like San Raffaele manage 300,000 patients every year. This massive volume means Italian surgical teams often prioritize high-turnover recovery protocols. Always secure flexible flight tickets because these high-volume centers may delay your discharge by 2 or 3 days to ensure complete cardiovascular stability.

Patient Consensus: Patients emphasize the need for a comfortable local apartment because hospital stays are often shorter than expected. Many note that having a dedicated translator helps navigate complex post-operative medication instructions before heading home.
