Rouzbeh Fazeli, Implantologist

Dr. Rouzbeh Fazeli is a specialist periodontist and implantologist in İzmir, Turkey, and a co-founder of Nifa Dent International. His clinical focus is minimally invasive implantology for patients with bone loss: planning implant treatment around the existing bone with short and narrow-diameter implants and crestal (internal) sinus lift, instead of extensive bone grafting where this is clinically appropriate. Consultations are available in English, Turkish, Persian and Azerbaijani.

Dental implants with bone loss – without bone grafting where possible

Many patients are told they need bone grafting, a lateral sinus lift or zygomatic implants before they can have dental implants. Dr. Fazeli's approach starts from the bone that is already there: the implant site is measured millimetre by millimetre on a 3D CBCT scan, and in suitable cases the treatment is planned with short or narrow-diameter implants, avoiding large grafting surgery, extra healing time and additional cost.

Short dental implants

Short implants (around 4–8 mm) are an evidence-based option when bone height is reduced in the back of the upper or lower jaw. They allow implant treatment in less bone, with less invasive surgery and a shorter overall treatment time compared with vertical bone augmentation.

Sinus lift without a lateral window (crestal / internal sinus lift)

When more height is needed in the upper back jaw, the sinus floor is gently elevated through the implant site itself using the osteotome technique – the subject of Dr. Fazeli's randomized clinical trial. This avoids opening a window in the side of the sinus and usually means less swelling after surgery.

Alternative to zygomatic implants

For a severely resorbed upper jaw, Dr. Fazeli does not recommend zygomatic implants that are anchored in the cheekbone. These cases are planned with short implants and minimally invasive surgery in the existing jaw bone; suitability is confirmed after examination and CBCT.

All-on-4 and All-on-6 full-arch dental implants

Fixed teeth for patients missing all or most of their teeth, using All-on-4 / All-on-6 protocols with tilted implants, flapless or computer-guided placement and, where possible, immediate placement and immediate loading of a temporary bridge.

Gum disease and periodontal treatment

As a specialist periodontist, Dr. Fazeli treats gum disease (periodontitis) with deep cleaning (scaling and root planing) and periodontal surgery, and corrects gum recession, crown length and gum contours with regenerative and plastic periodontal procedures – keeping natural teeth healthy and preparing a stable base for implants.

Peri-implantitis treatment

Diagnosis and treatment of inflammation and bone loss around existing dental implants, including surgical and regenerative management, to help preserve implants placed elsewhere.

Every plan is based on clinical examination and 3D CBCT imaging; the most suitable approach is decided for each patient individually. Patients who were previously told they lack enough bone for implants can send their existing panoramic X-ray or CBCT for a preliminary assessment before travelling.
