How Much Does Orchiectomy Cost in Spain?

Orchiectomy in Spain typically costs from $3,500 to $6,500 depending on the surgical technique and clinic tier. Patients save around 58% compared to the US, where the average price is $12,000. Most Spanish packages include the surgeon fee, anesthesia, one night of hospital stay, and initial follow-up consultations in cities like Barcelona and Madrid.

  • Radical orchiectomy: Often involves higher costs due to complex oncological diagnostics like PET/CT scans.
  • Prosthetic implants: Adding a testicular prosthesis typically increases the total procedure budget by over $2,100.
  • Gender-affirming care: Specialized multidisciplinary packages in Madrid may cost 40-50% more than standard medical procedures.
  • Clinic location: Facilities in major hubs like Barcelona often charge 15-20% more than regional centers.

Bookimed Expert Insight: Spain offers a unique balance of JCI-accredited safety and high-volume surgical expertise. Clinics like Centro Médico Teknon and Clinica Universidad de Navarra are ranked among the World's Best Hospitals by Newsweek. These centers perform over 23,000 operations annually, ensuring high proficiency. Choosing a JCI-accredited facility in Madrid or Pamplona often provides the same clinical standards as top US hospitals at nearly half the price.

The format is quite simple, everything is accessible and understandable, the doctor is very qualified. I will not say that we talked about clinical studies, there was no definitive information. We are happy, time has been spent, the translator also translated everything correctly. The doctor explained everything popularly, we received an answer to all the questions we asked. I believe that what you are doing is very important and useful for people who need help.

I saw Dr
From the minute I was inquiring about an appointment through to an entire 2 hours of tests, I felt completely taken care of. Being in a foreign country where it’s not your first language can be daunting when needing healthcare. My healthcare navigator Leen was so wonderful. She brought me to each doctor or department, got me situated and was also my translator. She was kind, compassionate & so incredibly helpful. I was incredibly grateful to have her by my side. I saw Dr. Bassols who was incredibly warm, knowledgeable and put my fears at ease. The other technicians were also wonderful and caring. I highly recommend the staff and care I received at Centro Medico Teknon and the team from bookimed that connected me to them. Thank you everyone!!

The doctor (Madero) didn’t even respond to our greetings when we entered his office. He didn’t even read correctly the medical report. He didn’t explained anything. He was doing things in a rush as if we didn’t have an appointment with him. He even left us in his office and went away. We were so frustrated after the consultation. Coming from Cameroon for this, was so disappointed.
About Bookimed service
Yes, Bookimed did a good job. After our misadventure with quironsalud Madrid, the medical coordinator booked us another appointment with Clinica Universidad de Navarra. The appointment went well and we were sarisfied of the hospital services.

I liked the doctor very much, it was clear that he was a professional in his field. Thanks a lot to Bookimed, and especially to my coordinating doctor Ekaterina Koliko.

FAQ about Orchiectomy in Spain

These FAQs come from real patients seeking medical assistance through Bookimed. Answers are given by experienced medical coordinators and trusted clinic representatives.

Can I have an orchiectomy under Spain’s public healthcare system?

Spain public healthcare system covers orchiectomy at zero direct cost for legal residents registered with the social security system. Legal residence, employment contributions, or specific reciprocal agreements like the S1 form are required to access care via a primary physician referral.

  • Eligibility criteria: Must hold a valid public health card from your local autonomous community.
  • Medical necessity: Surgery is covered for testicular cancer, prostate cancer, or severe physical trauma.
  • Gender-affirming care: Public health laws protect access to orchiectomy through regional Gender Identity Units.
  • Wait times: Public waiting lists for non-emergency or elective procedures often last several months.

Bookimed Expert Insight: Spain ranks 3rd globally for medical requests on our platform, illustrating its high surgical standards. While the public system is free, patients often choose JCI-accredited centers like Centro Médico Teknon or Hospital Ruber Internacional for faster access. Private facilities in Madrid and Barcelona handle 23,500+ operations annually, serving as a vital alternative when public waiting lists exceed several months.

Patient Consensus: Many patients recommend starting the referral process early due to bureaucratic delays. Those seeking gender-affirming care suggest documenting all hormonal history to streamline the multidisciplinarity evaluation required by public units.

What are the typical medical requirements for a gender-affirming orchiectomy in Spain?

Spain follows a model of informed consent and gender self-determination under the Law 4/2023. Patients require no psychiatric diagnosis or mandatory hormone therapy for gender-affirming orchiectomy. Primary requirements include being at least 18 years old and passing standard pre-surgical health evaluations for anesthesia safety.

  • Legal age: Patients must be at least 18 years old to provide legal consent.
  • Informed consent: Signing a document acknowledging the permanent sterility and risks is legally central.
  • Medical screening: Standard blood panels, electrocardiograms, and physical exams ensure safe surgery and recovery.
  • Condition management: Clinics require stable blood sugar levels, typically an HbA1c under 7% for safety.

Bookimed Expert Insight: Spain ranks third globally on Bookimed for patient requests, largely due to its depathologized legal framework. While state law doesn't require HRT, surgeons at top centers like Centro Médico Teknon often prioritize long-term surgical planning. They frequently adjust the incision technique to preserve scrotal skin for future vaginoplasty if a patient desires it later.

Patient Consensus: Patients value the dignity of the informed consent model but often face regional paperwork differences. Many recommend preparing a support letter from a therapist to streamline the process at private clinics.

Do international (non-EEA) patients need private care for an orchiectomy in Spain?

International patients typically choose private care for orchiectomy in Spain because the public system is reserved for legal residents paying social security. Private hospitals like Centro Médico Teknon or Hospital Ruber Internacional offer immediate scheduling, English-speaking staff, and streamlined documentation for medical tourists.

  • Residency requirements: Public healthcare requires legal employment, self-employment, or 5 years of residency.
  • Wait times: Private facilities bypass long public waiting lists for non-urgent or gender-affirming surgeries.
  • Cost efficiency: Private orchiectomy costs $3,500 to $6,500, saving up to 58% vs US.
  • Credentialed facilities: Top private clinics maintain Joint Commission International (JCI) and ISO International Standards.

Bookimed Expert Insight: While public care seems cheaper via the Convenio Especial after 1 year, medical tourists actually save more by choosing private centers in Madrid or Barcelona right away. These clinics often bundle pathology and follow-ups into a flat rate, which avoids the piecemeal billing common in Spanish state hospitals.

Patient Consensus: Patients emphasize that private care is the only predictable route for non-residents because public hospitals are not pay-and-go. They recommend confirming whether your surgeon performs the procedure as day surgery or requires overnight admission to accurately budget travel costs.

What does post-operative recovery and return-to-activity look like after an orchiectomy?

Recovery after an orchiectomy typically spans 2 to 6 weeks, with most patients returning to non-strenuous daily routines within 2 to 3 weeks. Success depends on following restricted physical activity guidelines to protect the incision site while managing localized swelling and initial discomfort.

  • Initial rest: Extensive rest and elevation are required during the first 48 to 72 hours.
  • Symptom management: Expect significant scrotal swelling, bruising, and a pulling sensation in the groin.
  • Work return: Sedentary tasks usually resume after 7 days; physical labor requires 2 to 3 weeks.
  • Activity restrictions: Avoid lifting over 10 pounds and sexual activity for at least 2 to 3 weeks.

Bookimed Expert Insight: Spanish centers like Centro Médico Teknon or Clinica Universidad de Navarra maintain elite standards with JCI and EFQM accreditations. High surgical volumes, such as 26,000 operations annually at HM Madrid, correlate with streamlined recovery protocols. Patients should prioritize clinics with integrated oncology departments for smoother post-operative transitions if hormone therapy is needed.

Patient Consensus: The first 72 hours often bring the most intense tightness and soreness. Patients emphasize that using ice and consistent scrotal support significantly eases the initial throbbing and local trauma.

How long should I plan to stay in Spain for an orchiectomy as an international patient?

International patients should plan to stay in Spain for 7 to 14 days for an orchiectomy. While the surgery lasts 30 to 60 minutes, you need time for pre-operative labs, wound healing, and obtaining a vital fit-to-fly certificate to prevent complications during travel.

  • Travel timeline: Arrive 2 days before surgery for blood tests and pre-anesthesia consultations.
  • Clinical stay: Most procedures are outpatient, though some clinics suggest a 1-night stay.
  • Flight safety: Surgeons require 10 to 14 days before flying to minimize thrombosis risks.
  • Physical limits: Avoid lifting any items over 5kg for 4 to 6 weeks post-surgery.

Bookimed Expert Insight: While basic recovery takes a week, Spain's top JCI-accredited centers like Centro Médico Teknon or Hospital Ruber Internacional often require a formal follow-up on day 10. Scheduling this check-up early is crucial because surgeons will not issue the required international travel clearance until they rule out hematomas.

Patient Consensus: Many patients find that the first 72 hours are the most uncomfortable for sitting or walking. They recommend booking airport assistance in advance, as handling even light luggage is physically demanding immediately after a bilateral procedure.

Will lifelong medication be required after bilateral orchiectomy?

Lifelong medication is required for most bilateral orchiectomy patients because removing both testicles eliminates 95% of natural testosterone production. This results in permanent surgical hypogonadism. Replacement hormones such as testosterone or estrogen are essential to maintain bone density, cardiovascular health, and emotional stability.

  • Hormone replacement: Restores energy and prevents osteoporosis through testosterone or estrogen therapy.
  • Medical indications: Most cancer or trauma patients require lifelong testosterone replacement therapy.
  • Gender-affirming care: Patients typically switch from testosterone blockers to lifelong estradiol therapy.
  • Prostate cancer: Replacement is usually avoided to prevent feeding hormone-sensitive cancer cells.

Bookimed Expert Insight: Spanish centers like Centro Médico Teknon or Hospital HM Nou Delfos often utilize multiphase follow-up plans. Our data indicates that because endogenous hormone fluctuations stop post-surgery, patients often achieve more stable blood levels than those with intact testicles. This allows for fewer dosage adjustments once the initial therapeutic maintenance level is established with your endocrinologist.

Patient Consensus: Many notice that while they feel better initially, fatigue and hot flashes can emerge if replacement is delayed. Regular lab monitoring of testosterone and hematocrit ensures long-term energy stays consistent.
